In order to evaluate the effectiveness and enjoyment level of each program, I would invite participants to complete an evaluation form in either electronic or hard copy format.  As an incentive for their feedback, I would offer them a coupon for $10 off any library fines they might have.  I would limit this to one coupon per person, per calendar year, and it must be used on their personal account.  In other words, a parent could not accumulate $30 in fine forgiveness for completing a form for him/herself and his/her two children.

In addition, I would have an internal form to be completed by any staff that was involved in the program.  This would be a short form (less than 3 minutes to complete) in electronic format that ascertains how they felt the program went, how it could have gone better, what changes we might implement for next time, etc.
